Mission's Health Education Center is a unique outpatient facility that empowers individuals with various health problems to better manage their own care. Specifically, our programs were developed for individuals with:
- High blood pressure and elevated cholesterol levels
- Special nutritional needs
Specially trained nurses and dietitians, plus a pharmacist and a behaviorist/certified clinical social worker, provide knowledge and support to help you optimize the quality of your life.
Our unique, collaborative care programs involve the education center, the individual, and his or her physician. Individuals availing themselves of the programs at Mission's Health Education Center may be self-referred, referred by their physician or referred by their employer.
The Health Education Center also coordinates programs with employers which have demonstrated excellent health outcomes. These Employer-Sponsored Disease Management Programs are available for people with diabetes, asthma, high blood pressure and elevated cholesterol. The education center forms a collaborative link with the individual, his or her physician, a community pharmacist or other healthcare professional (for ongoing follow-up) and the sponsoring employer (who provides financial support).
